It seems to vary: sometimes it's everybody and sometimes it's some sort of selectees. I haven't been able to find anything with a concrete listing. (There's a list of which airports have which sort of advanced imaging at which security checkpoints at FlyerTalk, which is interesting but different.)
I believe based on what I'm reading, but can't state as fact, that this is a combination of different behavior at different airports, and different behavior based on things like how crowded the line is.
If I come across anything more definitive I'll let you know.
